  1. Acyclovir versus valacyclovir

The aim of this Cochrane Review was to find out if valacyclovir performs better than acyclovir in the treatment of a painful itchy rash caused by the chickenpox virus (herpes zoster ophthalmicus). Cochrane researchers collected and analysed all relevant studies to answer this question and found one ophthalmicus is a painful itchy rash that appears on one side of the forehead. If the rash reaches the eye it may lead to visual impairment. This is because the infection can damage the front of the eye. is caused by the chickenpox virus, which can remain in the body for many years after the original chickenpox infection, and may get reactivated. Doctors can treat herpes zoster ophthalmicus with acyclovir. This is an antiviral medication that kills the chickenpox virus. After oral administration, bioavailability is as low as 15-30% [Table 1]. It is widely disseminated in the body fluids-cerebrospinal fluid, vesicle fluids, vaginal secretions and is excreted mainly through the kidneys, 85% in unmetabolized form. ACV crosses the placenta at all stages of pregnancy and is secreted into breast milk. ACV is a potent and selective inhibitor of herpes virus DNA replication. ACV is initially phosphorylated into ACV monophosphate by viral thymidine kinase, and not by the thymidine kinase of uninfected cells. ACV monophosphate is converted into ACV triphosphate by cellular (human) guanosine monophosphate (GMP) kinases and other kinases.

Description Valtrex (Valacyclovir hcl), the valine ester of acyclovir, is an antiviral agent active against herpes viruses, varicella-zoster virus, Epstein-Barr virus and to a lesser extent cytomegalovirus. Valtrex is used to treat genital herpes, orofacial herpes (cold sores), and herpes zoster (shingles). It is effective and safe for long-term suppression of recurrent genital herpes. Suppressive therapy is appropriate when outbreaks are frequent, severe, or emotionally disturbing. Valtrex is proven to reduce the risk of transmission of genital herpes. Valacyclovir suppresses shedding of HSV-2 on genital mucosal surfaces and may prevent sexual transmission of HSV. Valtrex effectiveness In herpes zoster, valacyclovir is more effective than acyclovir and equally effective as famciclovir in shortening the length and severity of shingles-associated pain and postherpetic neuralgia. Prompt treatment with valacyclovir can abort impending outbreak of both cold sores or genital herpes.
